Welcome to DI' NAMIK LAGREE STUDIOS LLC ("Studio," "we," "us," or "our"). By signing up for classes, attending sessions, purchasing packages, or using our facilities, you ("Client," "you," or "your") agree to the following Terms and Conditions. Please read carefully before participating in any services.

## 1. Eligibility
- All clients must be at least **18 years old** to participate in classes and use the facilities-**no exceptions**.
- By participating, you confirm that you are in good health and physically capable of engaging in Lagree fitness classes.

## 2. Class Reservations and Cancellations
- Class reservations must be made online through our website or booking platform.
- Each membership or class package may have a **specific cancellation policy**, including the cancellation window, applicable fees, or potential forfeiture of a class credit.
- **It is your responsibility to review the cancellation policy specific to your membership or package** at the time of purchase.
- If you fail to cancel within the required window stated in your plan, you may incur a **late cancel fee** or forfeit the class.
- **No-shows** may be charged the full cost of the class or have the class deducted from your package.
- Please arrive no earlier than 15 minutes prior to class, and no later than 5 minutes before the class start time. For safety reasons, late arrivals may not be admitted and will be treated as a no-show.

## 3. Memberships and Packages
- All purchases are final. Exceptions, if any, will be clearly stated at the point of sale or outlined in a written policy.**.
- Class packs have expiration dates as indicated at purchase.
- Monthly memberships auto-renew unless canceled in writing **at least 15 days** before the next billing cycle.
- Suspension or freeze policies may be available on a case-by-case basis and must be approved in advance.
Freeze requests may be granted for medical or travel reasons. Contact us in writing at least 15 days prior to your billing date to request a freeze.”

## 4. Studio Rules and Conduct
- Clients must wear **grip socks** and appropriate fitness attire.
- Personal items must be stored in designated areas; the studio is not responsible for lost or stolen items.
- Disruptive behavior, failure to follow instructor guidance, or disrespect toward staff or clients may result in suspension or termination of studio privileges.

Waiver and Release of Liability

By checking the box and submitting this form electronically, you are providing your electronic signature and agree to be legally bound by the terms of this Agreement.
This Waiver and Release of Liability ("Agreement") is made between you ("Participant") and DI'NAMIK LAGREE STUDIOS LLC, a Florida limited liability company with its principal place of business located at 11740 N. Dale Mabry Hwy., Tampa, FL 33618 ("Studio").
1. Eligibility to Participate
By acknowledging this waiver, you certify that you are 18 years of age or older. No person under the age of 18 is permitted to participate in any classes, sessions, or activities at the Studio. All participants are required to wear grip socks and appropriate fitness attire while engaging in any activity at the Studio.
2. Assumption of Risk
You understand that participating in Lagree fitness classes and using the Studio's equipment and facilities (the "Activities") involves inherent risks. These include, but are not limited to, physical exertion, muscle strain, sprains, fractures, equipment-related injuries, exposure to communicable diseases (including COVID-19), and other hazards that may result in serious injury or death. You voluntarily assume all such risks, whether known or unknown.
3. Health Declaration
You confirm that you are physically fit and have no medical condition that would prevent you from safely participating in the Activities. You acknowledge your responsibility to consult a physician before starting any exercise program and agree to immediately notify the Studio of any health changes that could impact your participation. You understand that the Studio is not a medical provider and does not assess your physical condition. It is solely your responsibility to determine whether you are fit to participate.
4. Release of Liability
In consideration of being allowed to participate, you hereby release and discharge DI' NAMIK LAGREE STUDIOS LLC and its owners, members, managers, officers, employees, contractors, and agents ("Released Parties") from any and all liability, claims, demands, or causes of action, including those based on negligence, arising out of or related to any injury, loss, or damage you may sustain while participating in the Activities.
5. Indemnification
You agree to indemnify and hold harmless the Released Parties from and against any and all claims, actions, damages, liabilities, costs, and expenses (including attorney's fees) resulting from your participation in the Activities.
6. Media Release
You authorize the Studio to use photographs, videos, and other recordings that may include your image or voice for lawful promotional and marketing purposes, without compensation.
7. Governing Law
This Agreement shall be governed by the laws of the State of Florida. Any legal action shall be brought exclusively in a court located in Hillsborough County, Florida.
8. Acknowledgment and Consent
By checking the acknowledgment box online, you confirm that you have read, understood, and voluntarily agreed to the terms of this Waiver and Release of Liability. You acknowledge that you are waiving certain legal rights, including the right to sue.
